FINANCIAL ASSISTANCE
Out of Network
Patients receiving treatment at Miami Surgery LLC under insurance that is out of network may be eligible to receive an adjustment to their assigned out of network patient liability, as long as our facility is not prohibited from offering Out of Network adjustments under state/Federal laws or your insurance company’s provisions. If this policy is not prohibited, applying any out of network discount is subject to patient’s benefit coverage. Delinquent accounts may have the adjustment disallowed.
Collection Procedures
In order to simplify our patients filing insurance claims, we will file an insurance claim on behalf of the patient to his/her insurance company. The patient must respond to his/her insurance company's request for information in a timely, as needed, manner to minimize processing delays with the claim.
Patients are expected to pay their financial obligations on time and any remaining portion of the charges will be paid upon finalization of the claim. Unpaid claims by the payer may result in the outstanding balance being fully transferred for collection.
If needed, the Miami Surgery LLC will attempt to reach a patient to secure payment on the outstanding balance using internal and external resources. In cases where the account becomes delinquent, it may be necessary to place with an attorney or agency for collection. The fees and expenses to collect the balance may become the obligation of the patient.
Other Providers
Services may be provided by Miami Surgery LLC at the facility as well as by other health care providers who on ocassion may bill the patient separately and who may or may not participate with the same health insurers or health maintenance organizations as the facility.
Current Patients and prospective patients may request from Miami Surgery TLC as well as other health care providers a specific personal estimate of charges and other information, with an explanation of billing practices. Patients should contact individual health care practitioners who will provide services in Miami Surgery LLC to understand what health insurers and health maintenance organizations that the health care practitioner participates as a network or preferred provider.
